deploy.status? OK validate.tests.passed? false (exit code 0 — no explicit failure)
Validation didn’t explicitly fail, so success was inferred. The system reported success because no stage asserted failure. This is the same failure mode as LLM overconfidence: absence of a negative signal treated as positive confirmation.
Recommendation (RFC):
Confidence is not correctness. Production doesn’t negotiate.
